Unusual sound, heat, oil leakage or a protection trip can indicate a developing problem. One symptom alone does not reveal the fault location or cause; site safety and event records come first.

This guide is general information. HV/MV intervention and testing must be carried out by qualified personnel under the equipment manual and facility safety procedures.

Signals worth investigating

Changes from normal sound or vibration, high temperature, reduced oil level, smoke, unusual odor and repeated breaker operation warrant prompt assessment. A relay trip may originate in the transformer, cable, breaker or protection chain.

  • Abnormal sound, vibration or smell
  • Oil or temperature changes
  • Gas and other protection events
  • Repeated trips or voltage issues

First, protect people

Keep unauthorized people away. Only qualified personnel should isolate, lock out and verify absence of voltage under site procedures. Preserve the time of the event and relay records; do not approach unsafe equipment to confirm a symptom.

Find the root cause

Inspection, event logs and selected insulation, ratio, winding and oil tests narrow the possibilities. Compare results with nameplate values and prior reports. After repair, verify relevant functions before controlled re-energization.

Distinguish a symptom from its cause

High temperature may be associated with loading, cooling or contact resistance; a thermal image alone does not establish the root cause. The trip time, relay target and other equipment events are useful early evidence. On oil-filled units, document level changes and leaks from a safe location. Qualified personnel should convert these observations into an appropriate test plan and a controlled decision on return to service.

Decision checklist

  • Event time and tripping protection
  • Safe observations of sound, smell, smoke and leakage
  • Recent load and temperature records
  • Previous maintenance and tests

Frequently asked questions

Does a relay trip prove the transformer has failed?

No. The transformer, cable, breaker and protection record must be assessed together.

What if there is smoke or a burning smell?

Secure the area, follow the facility emergency procedure and contact qualified responders.

Does unusual noise always mean an internal fault?

No. Fans, connections or external vibration can produce noise; event and measurement records help distinguish the cause.
